Did you email support in between? It's unlikely that someone would have just gone in and fiddled with your activations, but if you emailed support about it, I might have increased them for you.
Could you PM me with the order number, please?
Also, two things you should keep in mind - activating with the order number and password doesn't usually work for Season 1 customers (so don't bother trying it with any of the other episodes

), and if your recent upgrade included installation of Vista, Culture Shock is not going to stay activated for you (which could be why the other episodes activated automatically and episode 1 didn't). If you keep being prompted to activate and keep entering your order number and password, then yes, you will run out of activations eventually.
