Valve didn't have an online store until the Mannconomy Update in October 2010 (September 30th, actually, but less than 6 hours before October began). They had both promo items and hats available before that... the Sam and Max set being one obvious example.
However, they've been doing way more promo items as of late.

Misc items only have unusual versions if they were a hat that got converted into a Misc, and only then if it was after the Mannconomy update.
That's why you have unusual versions of the Le Party Phantom and Blighted Beak, but not of the Whiskered Gentleman or Camera Beard.
