The fact that De Singe has access to Elaine seemingly can mean that she is on the island with LeChuck. Why would LeChuck be there unless he has something planned. Perhaps he once went to Flotsam to get some monkeys and met De Singe there. Now he's using the monkies as yet another exuse to go to Flotsam.

Nice theory there, darknessofheart. Like De Singe said: "La mort es tué" - Death is beaten. It's most likely that the elixir will offer immortality to anyone who drinks (?) it. I actually didn't think about a link to LeChuck here, missing his former powers, referring to the scene where he's beaten up by the pox ridden pirates on Spoon Isle. That might explain why he's holding back his evil mind, playing nice 24/7 - one wrong step and he might die a human death which will be irrevocable. Perhaps this is also why he's treating Elaine so patiently during her episodes, she might as well attack him while under the pox (as Guybrush did with Bugeye).
Also, your words remind me of what Guybrush said to LeChuck in the opening scene of EP1: "If evil is always evolving - why doesn't it evolve in something less butt-ugly?" Well, there we go: LeChuck took his advice. ;-)
I still don't see why a nice, clean and handsome LeChuck would want a poxed Elaine as his bride. Maybe once De Singe is done with extracting the pox from her (IF he gets that far, that is) she will be freed from the pox?
I'm also curious as to how Elaine will fall into De Singe's hands - if she does (the synopsis just speaks of him having another subject in sight).
There are quite a few possible reasons why LeChuck and Elaine will go to Flotsam. Apart from the possibility of LeChuck having a deal with De Singe, the winds might just pull them in as they get too close to the island.
Or, since they didn't find Guybrush in Spinner Cay where they separated, they head for "that island he spoke of with the winds blowing inward."
Also, they might head for Flotsam for the fact that there is a doctor who might help Elaine with "whatever she has come down with." We saw that LeChuck is keeping Elaine in the dark about the fact that she's got the pox, nor does she seem to realize it herself. As far as she goes, she just doesn't feel too good and has that strange cough.
That is: She might go there voluntarily.
I can also imagine a scene of De Singe on his balcony, watching LeChuck and Elaine arriving at the docks and becoming witness of an outbreak of her pox. That being right after he realizes that Guybrush is cured, thus useless to him (and being whisked away to court anyway). He would then assign Morgan (still being around after delivering Guybrush to him) to go get Elaine.
We remember Morgan spitting out Elaine's name in disgust at the end of EP2. She has a reason to hate Elaine so she will gladly accept to take her in.
Whatever common past there might be of these two, it should be revealed in EP4. I personally dislike the idea of a girlfight in a Monkey Island game, but, well, I think a confrontation between Morgan and Elaine is inevitable (and I guess some of the boys here will enjoy a catfight scene between them).
All this being only a theory, of course. ;-)
