Can someone also confirm if decisions have a massive impact on the overall storyline?

as the game says in the opening, choices "tailor" the game to your style. It's a good analogy. The suit is the same suit, but the fit and cut are different.

Whether these make a massive impact would probably depend on your definition of what that means.

The same things will happen overall, despite the choices you make.

But what is said, how it is said, how characters feel about you and each other, who is present when, etc. These will be different.

Different choices definitely make a different game, even if the major events of that game are the same. imo
